List your employment history for the last 4 years. Include babysitting or work on a family farm as employment, even if you were not paid. Be sure to list total hours worked, not weekly or monthly averages. List in order from most recent employer.

List volunteer work or community service performed in the last 4 years. List in order from most recent work/service.

You must upload the following required documents.

• Statement of Intent and Use
  A brief typed statement of your post-secondary educational plans, and how you feel you merit consideration for this scholarship)
• Two (2) Letters of Recommendation
  Not to contain more than 150 words. One must be from a non-related adult, the second from a non-related teacher, instructor or employer. These should be written on letterhead so that they may be contacted for further information if needed.

Your application will be judged on planned course study. Factors such as accomplishments, community service and employment history are also considered while reviewing applications.
